|Animal Research Application||There are two forms that are BOTH required to be filled in for the research project. One is the administrative information (pdf) and the second one is the actual project description (Word). |

|Additional Team Members Form||Additional Team members form||Use this form to provide details of any additional staff you wish to include as part of your project application, should the details of those staff not fit on your original application form. If you are seeking to add personnel to an existing project, please use the Project Modification Request Form.|
|Monitoring Sheets - Examples||These are examples only and need to be modified to suit each project. The monitoring sheets intended to be used have to be submitted with the initial application and a sample of the filled in sheet will need to be submitted with the annual and final reports.|
|ACEC-R2||Annual Report and Continuation of animal use request form for Research projects||This form is used for the annual reporting on projects that have been approved for more than twelve (12) months as per forms ACEC-R1 or ACEC-W1. The annual report and request for continuation form has to be submitted to a full committee meeting before the authority expires.|
|ACEC-R3||End of Project Form for Research projects||To report at conclusion research projects or wildlife research projects approved by the ACEC.|
|ACEC-T1||Use of Animals for Teaching Purposes Application Form||General purpose application form for the use of animals for teaching purposes.|
|ACEC-T2||Annual Report and Continuation of animal use request form for Teaching projects||This form is used for the annual reporting on projects that have been approved for more than twelve (12) months as per forms ACEC-T1. The annual report and request for continuation form has to be submitted to a full committee meeting before the authority expires.|
|ACEC-T3||End of Project Form for Teaching projects||To report at conclusion of teaching and practical class fieldwork projects approved by ACEC.|
|ACEC-W1||Wildlife Research Application Form||Modified ACEC-R1 to suit wildlife studies in the field (especially survey type studies).|
|ACEC-AE||Unexpected Animal Event Report Form||To report unexpected event/death of an animal.|
|MODREQ||Project Modification Request Form||To request modification for an approved project.|
|Animal Use Consent Templates||These forms are designed for use by Principal Investigators of research projects that will involve privately-owned animals. There are two parts to the consent form, the 'Animal Use Consent Form' needs to be filled in and retained by researcher, the 'Animal Use Information Statement' has to be retained by the owner of the animal for their reference. The third form is if you require to use images/audio of the animals and need consent.|
